Are there any companies that carry the Phoenix belt that will bill my insurance for me? Yes, we work directly with the following medical supply companies that will verify your insurance benefits and obtain all the necessary paperwork from your physician and insurance company, then bill your insurance directly.

Parthenon does not take assignment. They will complete all the paper work for filing Medicare claims for your orders. Following that, Medicare will reimburse you directly.
If I have Medicare or private insurance, do I have to pay anything?
Yes. Apparel Med does not accept any 3rd party billing methods (Medicare or other insurance). Our customers must pay for their products via credit card, PayPal or Money Order. Customers may then submit their invoice as proof of payment for their goods to their respective insurance provider. Apparel Med makes no guarantees that the items the customer purchases are subject to reimbursement. Medicare has assigned a reimbursement code to the Phoenix Ostomy Support System. The code is: HCPCS 4396 (Ostomy belt with peristomal hernia support) which will be listed on your invoice.
What methods of payment do you accept?
We accept VISA, MasterCard, Discover and American Express. We also accept PayPal as well as Money Orders. We DO NOT accept personal checks.
What are my options for shipping within the US?
We ship from Washington State. Options and pricing (for 1 belt) are as follows: USPS 1st Class Mail=$4.18 (1-7 days); Priority Mail=$6.79 (1-4 days); or Express Mail=$21.49 (1-2 days).
Will you ship to post office boxes, APO, FPO addresses, Alaska, Hawaii and Puerto Rico?
Yes, we will. See above for S&H cost. Please allow additional time for any extra distance.
Do you ship internationally?

Do the belt’s removable inserts need to be removed before laundering?
No, the inserts can be washed and dried while remaining in the belt.
What is your return or exchange policy?
If for any reason you are not completely satisfied with your belt then please return it for a full refund within 2 weeks of receiving your belt. If you need to exchange your belt for a different size, please return it with a copy of your invoice and a note with your instructions. We will ship you out a new sized belt free of charge.
How do I determine which size to order?
You should measure around the body at stoma level to determine the best size for you.
Can you mail me a brochure and order form?
